The Mexico City Policy (MCP), initially established during Reagan’s administration in 1984, mandates that foreign NGOs must certify they do not perform or promote abortion to receive U.S. global family planning assistance. Expanded by the Trump administration, the MCP now encompasses most U.S. foreign aid, including extensive global health funding exceeding $7 billion. The policy, recently rebranded as “Protecting Life in Foreign Assistance” (PLFA), restricts not only abortion services but also the promotion of related activities through advocacy and public campaigns. Under the new framework of “Promoting Human Flourishing in Foreign Assistance” (PHFFA), set to take effect on January 27, 2026, further restrictions include prohibiting discrimination-related activities, diversity, equity, and inclusion (DEI) initiatives, and gender-affirming care. The MCP’s latest iteration is estimated to potentially impact up to $39.8 billion in foreign aid and around 2,600 organizations, profoundly influencing global health and development funding.
