Innovative Uses of AI in Musculoskeletal Radiology

A 2025 scoping review in BJR Open highlights the transformative role of artificial intelligence (AI) in musculoskeletal (MSK) radiology, led by Dr. Mickael Tordjman from Icahn School of Medicine. The review outlines AI applications across seven key areas: accelerated image acquisition, enhanced interpretation, large language models (LLMs), workflow integration, cost-effectiveness, liability concerns, and education. With early emphasis on fracture detection, AI has expanded to include deep learning-based reconstruction and automated classification of bone lesions, improving diagnostic accuracy and efficiency. Notably, AI applications can reduce scan times by up to 53% while maintaining image quality. LLMs streamline report generation, supporting communication among radiologists, clinicians, and patients. Successful AI adoption depends on seamless integration into existing systems, regulatory compliance, and proof of cost-effectiveness. Ultimately, AI serves best as an assistive tool, enhancing radiologist capabilities rather than replacing them. Clear guidelines are essential for safe, responsible implementation.
