Researchers at NYU Langone Health have developed an innovative AI model that predicts which patients will require skilled nursing care post-discharge, enhancing hospital discharge planning and alleviating stress for both patients and caregivers. This groundbreaking study, published in npj Health Systems, highlights the role of skilled nursing facilities in providing essential short-term care and rehabilitation for recovering patients.
Utilizing electronic health records from around 4,000 patients, the AI tool analyzes admission notes to extract critical information on seven risk factors, creating a concise ‘AI Risk Snapshot.’ The model demonstrates an impressive 88% accuracy rate in predicting the need for skilled nursing care, significantly outperforming traditional methods.
This proactive approach allows healthcare teams to coordinate care, arrange transportation, and prepare documentation, ensuring a smooth transition for patients while optimizing resources. Future work will focus on implementing this AI tool in clinical settings to further enhance patient care.
